Statement of Limiting Conditions and Appraiser's Certification (Form 1004B) Use of Form 1004B: Property appraisers use this form to evidence their understanding of Fannie Mae's definition of market value, to make certain required certifications, and to acknowledge contingent or limiting conditions to their appraisal reports. This form must be printed on legal size paper, using portrait format. Instructions: The appraiser must execute a Form 1004B and submit it with each appraisal report he or she prepares. The appraiser may not make any changes or deletions to this certification, although he or she may make additional certifications on a separate page or form. Acceptable additional certifications include those required by state law and those related to the appraiser's continuing education or membership in an appraisal organization. The appraiser may not add additional limiting conditions. |
